Aging and Disability Resource Connection:
Mercer County’s Aging & Disability Resource Connection is the doorway to information for seniors, adults with physical disabilities age 18 or older, family caregivers and private-pay consumers to find out what is available and help them access services quickly. This resource is the best way for consumers to find out about:

  • Home and community-based services to help consumers remain active and independent.
  • Medicare, Medicaid and other insurance options.
  • Planning for care needs now and for the future.
  • Housing and transportation options.
  • Caregiver assistance and supports.
  • Health promotion and disease prevention services
  • Work and volunteer opportunities.

Office for the Disabled:
The Mercer County Office for the Disabled funds programs and services that support those living with intellectual, cognitive, developmental and physical disabilities; their families; and caregivers. Information, assistance and referral is made to funded agencies as well as organizations that are designated to help those living with specific disabilities. Agencies receive funds from the Office for the Disabled through the Request for Proposals (RFP) process.

Personal Assistance Services Program:
The Personal Assistance Services Program assists those living with disabilities in Mercer County. Services differ from traditional home health services in that they are consumer-directed and supervised and are considered to be social services, rather than medical. Personal care is offered to assist consumers with continuing to thrive in the community of their choice.

T.R.A.D.E. (Transportation Resources to Aid the Disadvantaged and Elderly):
